Stray cats, TNR and coexistence: what the evidence supports

A cat beside a rubbish bin may be lost, abandoned, born outdoors or allowed to roam from a nearby home. A meal can relieve hunger, but it cannot by itself resolve injury, repeated breeding, wildlife predation or neighbours’ concerns. Caring about cats means taking all of these seriously.

This report accompanies PurrJung’s vision. It is a narrative literature review, not an original field investigation, a systematic review or a veterinary assessment. Sources were checked on 12 September 2026; the method and access limitations are explained below.

The findings in brief

  • Outdoor life is not a single welfare condition. Some cats receive dependable care; others face untreated injuries, disease and reproductive pressures. Neither “all are happy and free” nor “all are beyond help” describes them adequately. 1
  • TNR prevents reproduction in successfully sterilized individuals; population reduction is a separate outcome. Sustained, geographically coordinated programmes can reduce numbers, but abandonment, immigration and compensatory changes can offset progress. Neutering does not stop hunting. 5
  • Coexistence requires responsibilities, not just tolerance. Our recommendation is to combine prevention of abandonment, appropriate adoption, locally suitable TNR, ongoing care, public hygiene and wildlife protection, then measure whether harm actually falls. This is an ethical and practical recommendation, not a universal effect size established by one experiment.

Which cats are we talking about?

Here, “stray cats” is an everyday umbrella term, not a precise scientific category. Free-roaming domestic cats include owned cats with outdoor access as well as unowned cats. “Feral” usually describes cats with little socialization to people; it does not mean a native wildcat species. “Community cat” describes a care arrangement in some places, not proof that every resident has agreed to it. Definitions and legal responsibilities vary. 1 7

These groups are connected. Births, abandonment and lost pets add cats; reunification, adoption, deaths and movement remove them from a particular count. A fearful cat may be a lost pet, and a friendly cat may already have a home. Appearance or one encounter cannot establish ownership or suitability for adoption. This is why management must include owned cats and support for their carers, rather than treating street populations as isolated. 1

Life outside: vulnerability without stereotypes

The most useful welfare questions concern the individual: can it eat and drink reliably, rest safely, move without pain and receive treatment when injured? Traffic, conflict, weather exposure and untreated illness can matter even where food is available. Repeated reproduction also creates more kittens needing care. Conversely, some managed outdoor groups have reasonable welfare; seeing a cat outside is not enough to diagnose suffering. 1

A frequently cited cohort study reported that 127 of 169 kittens (75%) died or disappeared before six months of age. “Disappeared” is not “confirmed dead.” This local study of free-roaming cats, published in 2004, is evidence of serious early-life vulnerability in that setting, not a global kitten mortality rate or a basis for saying all stray cats live only two or three years. We checked its public abstract, not the full paper. 2

Nor is taking every cat indoors automatically a good outcome. Socialized cats may benefit from suitable homes; poorly socialized cats may experience substantial confinement stress, and overcrowded shelters have welfare limits. The relevant comparison is between feasible, adequately resourced care options, not an idealized street and an idealized shelter. Individual health and behaviour need assessment by veterinary and experienced welfare teams. 1

Wildlife and public health belong in the same discussion

A 2023 global review documented 2,084 species consumed by free-ranging domestic cats, using 533 publications with species-level records. This measures diet breadth, not annual deaths or the number of species driven extinct. Some records may involve scavenging rather than confirmed predation. Nevertheless, the breadth of documented consumption makes it unreasonable to assume that outdoor cats affect only rodents. 3

China also has relevant research. Li and colleagues used 2,187 questionnaire responses and statistical extrapolation to estimate wildlife killed by free-ranging cats, including owned outdoor cats and unowned cats. This is not a direct national census. Respondent selection, recall, unseen prey and extrapolation introduce uncertainty; national estimates cannot diagnose the impact in one housing estate. The study’s public abstract and introductory material were available to us, not its paywalled full text. 4

The consequences depend on the prey species, habitat and population vulnerability. An island breeding colony, a protected wetland and a built-up neighbourhood are not interchangeable. A diet record is not itself proof of population decline, but uncertainty is not a reason to wait for the loss of a vulnerable species before protecting its habitat. That last principle is our precautionary recommendation. Food provision and sterilization do not guarantee that cats stop hunting. 3 5

Health risks also need accurate framing. Bites, scratches and contact with contaminated faeces or soil can create exposure, but being near a cat does not by itself mean infection. Toxoplasmosis also has foodborne routes, including undercooked meat; portraying it simply as a “stray-cat disease” misrepresents prevention. Handwashing, gloves for cleaning, prompt faeces removal, covered sandboxes and veterinary vaccination and parasite plans are useful safeguards. Pregnant or immunocompromised people should seek individualized clinical advice and avoid handling cat litter where possible. 8 9

After a bite, scratch or other possible rabies exposure, promptly wash the wound with soap and running water for at least 15 minutes and seek urgent medical assessment. Do not wait for symptoms or decide that treatment is unnecessary because the cat looks well. Clinicians assess wound infection, tetanus and the need for rabies post-exposure care under local guidance. 8 10

What TNR means—and why it matters

TNR stands for trap–neuter–return: humane capture, veterinary sterilization and return to the original location when appropriate. Responsible programmes also arrange health assessment, vaccination as locally indicated, identification such as ear-tipping under anaesthesia, postoperative care and continued monitoring. Return is a decision after assessment, not an obligation to release every cat regardless of danger. 1 7

Its clearest significance is preventing further litters from sterilized cats. It can also reduce some reproduction-related behaviours and provide access to care for cats who would otherwise receive none. At a community level, it offers a way to involve existing carers in preventing future births while caring for cats already alive. These are meaningful aims even though individual welfare, fewer births, fewer cats and less wildlife harm are different endpoints. 1 5

Capture, transport, anaesthesia and surgery also carry stress and medical risks. Veterinary teams must determine suitability, pain relief, recovery requirements and what to do for pregnant, nursing, sick or injured cats. This report is not a trapping or surgical manual. A single operation does not cure an infection, make a return site safe or guarantee lifetime vaccination coverage. 1 8

Does TNR reduce numbers? Read the intervention and the denominator

A city study: coverage must extend beyond one colony

Gunther and colleagues followed a phased programme in Rishon-LeZion, Israel, during 2007–2018. During partial-city treatment, the most heavily treated comparison group reached about 80% neutering, yet its cat counts did not decline significantly. The authors proposed immigration from surrounding untreated areas as an explanation. 5

After high-intensity treatment expanded across the city, neutering reached about 72% in the surveyed area. Counts on the study’s survey routes fell from 4,062 in 2014 to 3,122 in 2018—a 23.1% reduction. These are comparable survey counts, not a census of every cat in the city. The long-term, spatial comparison is more informative than a one-off before-and-after photograph, but it is one urban setting, not a randomized comparison across cities. Detection bias and other changes over time remain limitations. 5

The study also found changes consistent with compensatory reproduction and survival. Its lesson is not “70% always works,” but that intensity, continuity, neighbouring populations and ongoing births all matter. Higher survival is good for individual cats, even when it slows the decline in population size. 5

A campus study: TNR plus adoption is not TNR alone

A Florida university campus programme followed cats during 1991–2002. The population counted declined from 68 in 1996 to 23 in 2002, approximately 66%. Across the whole study, 155 cats were enrolled and 47% were adopted; other outcomes included death, disappearance, movement and euthanasia. It is incorrect to describe this as a census falling from 155 to 23, or to assign all of the decline to sterilization. 6

This uncontrolled, single-site programme supports the possibility of sustained reduction through combined management, not a promise that occasional sterilization alone will reproduce the result. We checked the public abstract. 6

Reviews: encouraging examples, uneven evidence

A 2024 systematic review retained 11 studies, seven from the United States; all used TNR. Greater recorded community involvement, especially adoption and education, accompanied better population outcomes. But involvement was measured simply by reported roles, the geographic coverage was narrow, and the review excluded culling. It does not establish a causal effect of education alone, a global TNR success rate or the superiority of TNR over every alternative. 7

The 2025 UK-focused review likewise favours coordinated management while acknowledging gaps. Across this literature, varying definitions, few controls, combined interventions, limited follow-up and possible publication bias make direct comparison difficult. Evidence supports conditional benefits, not either blanket claim that TNR always works or never works. 1 5

How should people coexist with these cats?

The following is PurrJung’s proposed approach, informed by the sources—not a field-tested protocol or legal advice. The aim is fewer preventable harms to cats, wildlife and people, with responsibilities shared beyond individual volunteers.

  1. Prevent new arrivals at the source. Support affordable veterinary sterilization, identification and reunification, escape prevention, behaviour help and temporary care when owners face difficulty. Keep owned cats from roaming unsupervised, with enrichment and safe enclosed or supervised outdoor options. Abandonment prevention and supported rehoming matter as much as street-level intervention. 1
  2. Assess before choosing a destination. First check for an owner through local lost-pet procedures and professional microchip scanning. Arrange suitable adoption or foster care for cats who can benefit. Cats needing treatment require veterinary assessment. For poorly socialized cats, assess both the welfare costs of confinement and the safety, legality and care capacity of any proposed return. 1
  3. Use TNR only where a sustained programme is feasible. Confirm local rules, veterinary capacity, recovery space, carers, funding and geographic coordination. Include newcomers and follow-up; do not count an isolated sterilization day as population control. Where a site is dangerous or ecologically sensitive, work with relevant authorities and welfare and conservation specialists on removal and suitable placement rather than routine return. Do not relocate cats casually to another neighbourhood or natural area. 1 5
  4. Make feeding part of care, not the whole plan. Agree on suitable feeding places and times away from wildlife-sensitive areas and shared play spaces, provide adequate food, and remove leftovers and litter. Record the cats using each site and link feeding to sterilization and health monitoring. Unmanaged food and accessible rubbish can support more cats. Adjust provision as the managed population declines; suddenly starving dependent cats is not a humane management strategy. 1
  5. Give neighbours a say and a contact. Carers, residents who do not want contact with cats, property managers, veterinarians, public-health staff and wildlife specialists need a way to report concerns and share decisions. Agree who cleans, pays for treatment and responds when a volunteer is unavailable. Publish expenditure and outcomes without exposing private addresses or vulnerable colony locations. Community participation is useful, but unpaid carers cannot replace public responsibility. 7
  6. Protect welfare during difficult decisions. Neither poisoning nor abandonment in another place is an acceptable response to conflict. For an animal experiencing severe suffering, treatment, prognosis and any euthanasia decision belong with qualified veterinary professionals under local law. This is not the same as treating every unowned or inconvenient cat as a candidate for euthanasia. Evidence about population control alone cannot decide these ethical questions. 1

“Coexistence” does not require cats to remain in every outdoor location forever. A successful programme may mean fewer cats living without dependable homes or care, suitable cats entering homes, sensitive habitats protected and remaining cats receiving monitored care. Affection for cats and concern for birds or other small animals need not be opposing commitments.

Measure outcomes, not just operations

Before intervention, establish a defined area and repeatable baseline. Our proposed community record includes observed cats and kittens, known sterilization status, new arrivals, adoptions, confirmed deaths, disappearances recorded separately, injuries, complaints and local wildlife concerns. Keep observation routes, effort and season as comparable as possible, and record changes in detection conditions. An ear-tip count among easily approached cats can overestimate coverage if shy cats are missed. 5

An example planning rhythm is a quarterly operational review with six- and twelve-month outcome comparisons, followed by multi-year follow-up. These intervals are a planning suggestion, not a research-derived threshold for success. A fall in shelter admissions, fewer complaints or a large number of operations cannot alone prove that the outdoor population or wildlife harm has fallen. If births, arrivals or harms persist, revise the coverage, care arrangements, source prevention or location strategy rather than declaring success from effort alone. 1 5

Method, uncertainty and reading further

This review used targeted searches for free-roaming and stray-cat welfare, TNR population outcomes, wildlife impacts and community participation, then checked original study pages and CDC/WHO guidance. Selection favoured traceable methods, longitudinal evidence and explicit limitations; the ten sources below include primary studies, reviews and public-health guidance. This was not an exhaustive database search, meta-analysis or formal certainty grading, and no interviews or original cat counts were conducted.

Full articles were consulted for references 1, 3, 5 and 7, public abstracts for 2 and 6, and the public abstract and introductory material for 4; references 8–10 are institutional web guidance. We did not independently reanalyse study datasets or verify every paper cited within a review. The cited sources are in English, even when discussing China, and the evidence is geographically uneven. Translating this report does not turn overseas findings into local legal or veterinary guidance.

The report cannot establish the current global or Chinese stray-cat population, a universal lifespan, a guaranteed sterilization percentage, or the best intervention for an unassessed local site. Several studies are old; reviews synthesize evidence with different aims, and some authors work in animal-welfare organizations. Those perspectives and possible selection or publication biases warrant attention without automatically invalidating findings. We do not claim veterinary review or endorsement by the cited institutions.
